چکیده

Four men with arteriosclerosis obliterans (ASO) were exercised on a treadmill on 4 different days to determine their maximal walking time (MWT) and the point at which their claudication pain (CPT) began. Each subject was exercised at 60%, 75%, and 90% of his MWT to determine the effect of work intensity on the systolic blood pressure at the ankle (AP). To measure the effect of exercise repeated on the same day, each subject walked five times each day on 4 different days at each of the three workloads. After each subject completed the required number of walks, his MWT was again measured on 4 separate days (one subject was retested only once) to determine whether it had changed. In every case the greatest reduction in AP occurred after the initial maximal walk but the submaximal intensities caused different responses in the different subjects. With repetition of exercise each subject displayed a progressive increase in either the AP level taken 2 minutes postexercise or the rate of recovery of AP to the pre-exercise level. There were significant increases in MWT, in CPT, and in the resting and postexercise AP levels in some of these subjects. This was most probably due to increased collateral circulation before, during, and after exercise.

Exercise Physiology Efficacy of Quantified Home-Based Exercise and Supervised Exercise in Patients With Intermittent Claudication A Randomized Controlled Trial

Background—This prospective, randomized, controlled clinical trial compared changes in exercise performance and daily ambulatory activity in peripheral artery disease patients with intermittent claudication after a home-based exercise program, a supervised exercise program, and usual-care control. Methods and Results—Of the 119 patients randomized, 29 completed home-based exercise, 33 completed...

Exercise prescription using the heart of claudication pain onset in patients with intermittent claudication

OBJECTIVE To assess the acute metabolic and cardiovascular responses to walking exercise at an intensity corresponding to the heart rate of claudication pain onset and to investigate the effects of a 12-week walking training program at this intensity on walking capacity. METHODS Twenty-nine patients with intermittent claudication were randomly allocated to the walking training (n=17) or contr...

Medical treatment of peripheral arterial disease in the elderly

Smoking should be stopped and hypertension, diabetes mellitus, dyslipidemia, and hypothyroidism be treated in elderly patients with peripheral arterial disease (PAD). Statins reduce the incidence of intermittent claudication and improve exercise duration until the onset of intermittent claudication in persons with PAD and hypercholesterolemia. Antiplatelet drugs such as aspirin or clopidogrel, ...

An exercise test in intermittent claudication.

In most cases of intermittent claudication the diagnosis is apparent after the history and clinical examination have been completed. Although the history itself may be misleading, clinical examination is usually fruitful; the signs of obliterative arterial disease are present in most, and cases of anrmia and coarctation of the aorta that present with intermittent claudication should be easily d...

Walking exercise in patients with intermittent claudication. Experience in routine clinical practice.

BACKGROUND In patients with intermittent claudication, exercise in the form of walking is effective in reducing pain and maximising achievable walking distance. However, data are lacking on the implementation of walking exercise in these patients. AIMS To explore the current behaviour and views of patients with intermittent claudication towards taking walking exercise. DESIGN OF STUDY Posta...

Intermittent claudication: cost-effectiveness of revascularization versus exercise therapy.

PURPOSE To compare the costs, effectiveness, and cost-effectiveness of alternative treatment strategies for intermittent claudication. MATERIALS AND METHODS By combining data from the literature and original patient data, a Markov decision model was developed to evaluate the societal cost-effectiveness. Patients presented with previously untreated intermittent claudication, and treatment opti...
